This application assists in ensuring safe patient hand offs. This application is uniquely designed to provide essential information for optimal transition of care. Unlike most existing software solutions which only provide essential information required for patient care this application also provides tools to assign tasks for covering physicians. Monitoring tools are also built in to assess the efficiency of covering teams. Important information is structured so that it is readily visible as teams change especially when covering weekends. Important alerts like allergies, high risk medications are also highlighted. The application also allows voice memos and digital images to enhance communication between physicians.
